I don't think Oldham writes that one cant travel easlily on the continuum between healthy-unhealthy (he has seen many disordered people in clinical practice), However there is a realistic range/potential of development on the continuum for everybody, as well as the possibility to dim or boost other styles in your behavior situationally.
You are right on the psychopath issue. Millon's chapter on the anti social personality would give you the info you probably want to know. The book called Personality Disorders in Modern Life is really value for money when you want to go deeper than Oldham.
